Job summary

Visiting Nurse Health System, a trusted home health provider in the Atlanta area, seeks an LPN to join our Marietta Home Health team. You will deliver compassionate nursing care, educate patients and families, and collaborate with RNs to ensure accurate orders and continuous care.

The role is a full-time, on-site position with weekend flexibility and a strong emphasis on patient dignity and comfort in home settings.

Qualifications

• LPN graduate from an accredited school of nursing or program.
• IV training or IV therapy certification.
• Experience with advanced wound therapies.
• Current Georgia LPN license.

Responsibilities

• Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
• Provide nursing services as ordered by the physician and under the direction of the RN.
• Report subjective and objective patient findings to the RN.
• Under the direction of the RN, obtain physician’s orders and ensure orders exist for each visit.
• Document pertinent information and care rendered to ensure continuity of care.
• Demonstrate infection control standards per VNHS policy.

In this position...

• Provides nursing services as ordered by the physician and under the direction of the RN.
• Reports subjective and objective patient findings to the RN.
• Under the direction of the RN, obtains physician’s orders and ensures orders exist for each visit made,
• Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care.
• Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.

Do you have....

• LPN Graduate from an accredited school of nursing or program
• Completion of an IV training program or certification in IV therapy
• Experienced in advanced wound therapies
• Current license by the Georgia State Board of Nursing.
• Minimum of one - two (1-2) years’ experience in a Medicare-certified home health agency is preferred.
• Minimum of two (2) years as an LPN
• Current valid Driver’s License, proof of auto insurance, and a reliable vehicle
