Central Gardens Post Acute in San Francisco (Fillmore District) is seeking a PRN Physical Therapist Assistant (PTA) to join an in-house therapy team. You will deliver PT interventions under supervision, help residents improve mobility, and participate in interdisciplinary care.

Key Responsibilities

Treatment & Resident Care

• Deliver high-quality physical therapy interventions under the direction and supervision of a licensed Physical Therapist.
• Provide individualized treatment focused on mobility, strength, balance, functional independence, and quality of life.
• Monitor resident responses to treatment and communicate progress or concerns to the supervising PT.

Documentation & Progress Monitoring

• Maintain accurate and timely clinical documentation.
• Track resident progress and support achievement of established treatment goals.
• Follow facility policies, clinical standards, and applicable regulatory requirements.

Interdisciplinary Collaboration

• Collaborate with Nursing, Activities, and the broader Therapy team to support comprehensive resident care.
• Participate in care planning and communicate effectively with interdisciplinary team members.

Resident & Caregiver Education

• Educate residents and caregivers on safe mobility techniques, exercises, and strategies to promote independence.
• Support caregivers and staff with carryover of appropriate therapeutic and restorative techniques.

Qualifications

• Current, valid California Physical Therapist Assistant (PTA) License required.
• Strong communication and interpersonal skills.
• Team-oriented mindset with a passion for working with residents in a skilled nursing and post-acute setting.
