Job summary

San Francisco Post Acute seeks a compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team on a part-time basis. Help residents improve mobility, restore strength, and regain independence in a supportive post-acute setting.

The PTA will provide treatments under the supervision of a licensed Physical Therapist, document progress, and collaborate with nursing and interdisciplinary staff.

Qualifications

• Active California PTA license is required.
• Excellent communication and teamwork skills.
• Resident-centered approach to rehabilitation.

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Help residents improve strength, balance, coordination, mobility and functional independence.
• Implement individualized treatment plans and therapeutic interventions.
• Monitor resident responses to treatment and communicate progress to the supervising PT.
• Maintain accurate, timely, and compliant clinical documentation.
• Educate and encourage residents throughout the rehabilitation process.
• Collaborate with nursing and interdisciplinary team members to support resident-centered care.
• Follow facility policies, clinical standards, and applicable regulatory requirements.
