Job summary

Supplied Talent is seeking a dedicated and licensed PTA to join a rehabilitation team at a skilled nursing facility serving Ordway, Colorado. Under supervision of a Physical Therapist, you will deliver individualized therapy to long-term residents and post-acute patients, supporting recovery and quality of life.

Ideal candidates hold Colorado PTA licensure, an Associate degree, CPR/BLS certification, and have strong communication.

Qualifications

  • Associate’s degree from an accredited PTA program.
  • Current Colorado PTA license or ability to obtain before start.
  • Current CPR/BLS certification.
  • Strong interpersonal and communication skills with a patient-first mindset.

Responsibilities

  • Implement individualized physical therapy treatment plans as directed by the supervising PT.
  • Deliver therapeutic interventions including exercise, gait, balance, transfer, and modalities.
  • Monitor and document patient progress in accordance with standards.
  • Communicate with the supervising PT and interdisciplinary team.
  • Educate patients and families on home programs, fall prevention and safe mobility.
  • Participate in care planning meetings and MDS documentation.
  • Maintain accurate clinical documentation per Medicare/Medicaid and facility requirements.
  • Uphold resident rights, dignity, and confidentiality.
  • Maintain a clean, organized therapy gym and treatment area.

POSITION OVERVIEW

We are seeking a dedicated and licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. Reporting to the Director of Rehabilitation, the PTA will deliver individualized physical therapy services to long-term care residents and skilled nursing patients, supporting functional recovery and quality of life under the supervision of a licensed Physical Therapist.

KEY RESPONSIBILITIES
  • Implement individualized physical therapy treatment plans as established and directed by the supervising Physical Therapist
  • Deliver therapeutic interventions including therapeutic exercise, gait training, balance activities, transfer training, and modality application
  • Monitor and document patient progress, functional status, and response to treatment in compliance with state, federal, and facility standards
  • Communicate regularly with the supervising PT, interdisciplinary team, and nursing staff regarding resident goals and progress
  • Educate patients, families, and caregivers on home exercise programs, fall prevention, and safe mobility techniques
  • Participate in care planning meetings, utilization reviews, and MDS documentation as applicable
  • Maintain timely, accurate clinical documentation in compliance with Medicare/Medicaid and facility requirements
  • Uphold resident rights, dignity, and confidentiality in all interactions
  • Assist in maintaining a clean, organized, and safe therapy gym and treatment environment
QUALIFICATIONS
Required:
  • Associate’s degree from an accredited PTA program
  • Current, unrestricted PTA licensure in the State of Colorado (or ability to obtain prior to start date)
  • Current CPR/BLS certification
  • Strong interpersonal and communication skills with a patient-first mindset
Preferred:
  • 1+ years of experience in a skilled nursing, long-term care, or post-acute rehabilitation setting
  • Familiarity with MDS/PDPM documentation frameworks
  • Experience with geriatric populations and fall prevention programs
  • Knowledge of EMR/documentation systems used in SNF environments
