Job summary

Centra Healthcare Solutions Inc. is seeking a skilled or newly graduated Physical Therapist Assistant (PTA) to join a Caring SNF/LTC team in Sunnyvale, California. This direct hire, full-time position offers daytime hours and requires licensure in the state and CPR certification.

The PTA will deliver skilled services under the supervision of a licensed PT, implement and adjust the treatment plan, and work with an interdisciplinary team to reach patient goals.

Qualifications

• Requires an Associate degree from an accredited program.

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Physical Therapist Assistant / Physical Therapy Assistant / PTA to join their growing team! This is a direct hire / 'perm' position. Days schedule. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements

• Must have earned an Associates degree from an accredited school.
• Current state license as a Physical Therapist Assistant / PTA or proactively in the process of the application process for current state licensure as a Physical Therapist Assistant / Physical Therapy Assistant / PTA.
• Current CPR certification.
• Provide skilled physical therapy services / interventions in accordance with physician orders and under the supervision of a Registered or Licensed Physical Therapist / PT.
• Implement and modify the patient's physical therapy plan of treatment under the supervision of a Registered or Licensed Physical Therapist / PT.
• Effectively collaborates within an interdisciplinary team to meet patients goals.
