Job summary

Centra Healthcare is seeking an experienced or newly graduated Physical Therapist Assistant for a contract assignment in a SNF/LTC setting. You will work under the supervision of a PT and deliver skilled PT services per physician orders.

The role emphasizes collaboration within an interdisciplinary team, adherence to state licensure requirements, and the ability to implement and adjust treatment plans to meet patient goals.

Qualifications

• Must have earned an Associates degree from an accredited school.
• Current state license as a Physical Therapist Assistant / PTA or actively pursuing licensure.
• Provide skilled physical therapy services under supervision of a Registered or Licensed PT.

Responsibilities

• Provide skilled physical therapy services/interventions per physician orders under supervision.
• Implement and modify the patient’s PT plan of treatment under supervision.
• Collaborate within an interdisciplinary team to meet patient goals.

• Implement and modify the patient's physical therapy plan of treatment under the supervision of a Registered or Licensed Physical Therapist / PT.
• Effectively collaborates within an interdisciplinary team to meet patients goals.
