Job summary

Sunrise Home Health Services is seeking a compassionate Physical Therapist Assistant to join our Northwest Phoenix Valley team providing in-home therapy across Anthem, Surprise, Peoria and surrounding communities.

Under the supervision of a licensed Physical Therapist, you will deliver individualized home health treatments, monitor progress, educate patients and caregivers, and document in accordance with Medicare guidelines. Arizona PTA license and CPR are required; new graduates welcome.

Qualifications

• Must be graduate of an accredited Physical Therapist Assistant program.
• Licensed as a Arizona PTA.
• CPR certification or ability to obtain.
• Valid Arizona driver's license.
• Reliable transportation.
• New graduates are encouraged to apply.

Responsibilities

• Provide individualized physical therapy treatments in patients' homes.
• Implement treatment plans to improve mobility, strength, balance, endurance and independence.
• Monitor progress and communicate changes with supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Complete accurate documentation in accordance with Medicare and agency guidelines.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to deliver outstanding patient care.
