Job summary

Sunrise Home Health Services in Arizona seeks a compassionate Physical Therapist to join our rehabilitation team serving Northwest Phoenix, Peoria, Glendale, Sun City, Surprise and surrounding communities. You will perform home-based PT evaluations, develop individualized plans, and provide evidence-based interventions to improve mobility and independence.

We offer full-time or PRN positions with flexible scheduling, a dedicated territory, mileage reimbursement, and ongoing education.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Arizona Physical Therapist license.
• Current CPR Certification.
• Valid Arizona driver's license.
• Reliable transportation.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient goals and physician orders.
• Provide evidence-based therapeutic interventions to improve mobility, strength, balance, endurance, and function.
• Complete Start of Care assessments and ongoing therapy visits.
• Assess patients' home environments and recommend equipment or safety modifications.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate closely with physicians, nurses, occupational therapists, speech therapists, medical social workers, and home health aides.
• Complete timely and accurate electronic documentation.
• Help patients safely recover while reducing hospitalizations and maximizing independence.
