Job summary

Sunrise Home Health seeks a licensed Physical Therapist Assistant to provide in-home physical therapy under supervision in the Surprise, AZ area.

You will implement treatment plans to improve mobility, balance, and independence, educate patients and caregivers, document outcomes per Medicare guidelines, and collaborate with a multidisciplinary team. Flexible full-time or part-time scheduling with mileage reimbursement and a supportive leadership team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Arizona PTA license.
• CPR certification (or ability to obtain).
• Valid Arizona driver's license, reliable transportation.
• Home health experience preferred.

Responsibilities

• Provide individualized physical therapy treatments in patients' homes.
• Implement treatment plans focused on improving mobility, strength, balance, endurance and independence.
• Monitor progress and communicate changes with the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Recommend assistive devices and home safety modifications.
• Complete accurate documentation in accordance with Medicare and agency guidelines.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to deliver outstanding patient care.

Your Role

Working under the supervision of a licensed Physical Therapist, you will:
