Job summary

Sunrise Home Health Services in the Northwest Phoenix Valley seeks compassionate Physical Therapist Assistants to join our home health team. This role serves Anthem, North Phoenix, Peoria, Glendale, Surprise, Sun City, Sun City West and surrounding communities with flexible full-time or part-time scheduling.

Under supervision of a licensed Physical Therapist, you will provide in-home PTA care, implement plans to improve mobility, educate patients and caregivers, and complete Medicare-compliant

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Arizona PTA license.
• CPR Certification (or ability to obtain).
• Valid Arizona driver's license.
• Reliable transportation and current auto insurance.

Responsibilities

• Provide in-home physical therapy treatments under supervision of a licensed Physical Therapist.
• Implement treatment plans to improve mobility, strength, balance, endurance and independence.
• Monitor progress and communicate changes with supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Recommend assistive devices and home safety modifications.
• Complete accurate Medicare-compliant documentation.
• Collaborate with physicians, nurses, therapists and other health professionals.

Education

Your Role

Working under the supervision of a licensed Physical Therapist, you will:

• Provide individualized physical therapy treatments in patients' homes
• Implement treatment plans focused on improving mobility, strength, balance, endurance, and independence
• Monitor progress and communicate changes with the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs and fall prevention
• Recommend assistive devices and home safety modifications
• Complete accurate documentation in accordance with Medicare and agency guidelines
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to deliver outstanding patient care

Qualifications

Required
