Job summary

Sunrise Home Health Services in the Northwest Phoenix Valley seeks compassionate Physical Therapist Assistants to join our team delivering in-home therapy services. We offer flexible full-time or part-time opportunities with support from a skilled clinical leadership.

You’ll provide therapies to improve mobility and independence, coordinate with a multidisciplinary team, and enjoy competitive compensation with mileage reimbursement and benefits.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Arizona PTA license.
• CPR Certification (or ability to obtain).
• Valid Arizona driver's license.
• Reliable transportation and current auto insurance.

Responsibilities

• Provide individualized physical therapy treatments in patients' homes.
• Implement treatment plans focused on mobility, strength, balance, endurance and independence.
• Monitor progress and communicate changes with the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Recommend assistive devices and home safety modifications.
• Complete accurate documentation per Medicare and agency guidelines.
• Collaborate with physicians, nurses, therapists and other healthcare professionals.
