Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing when needed.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current Physical Therapist Assistant (PTA) License to practice in California – Required.
• Experience in Home Health care – 1 Year Required.
• Current CA Driver’s License.
• Able to travel within a 15‑25 mile radius on a regular basis.
