ABL Health Care in Pittsburg, CA is seeking a Full-Time Physical Therapist Assistant (PTA) to join our Home Health team. You will assess patients, develop plans, and support mobility and quality of life in the home setting.

As part of our interdisciplinary team, you’ll document progress, travel within a 15-25 mile radius, and enjoy comprehensive benefits including health, dental, vision, and retirement options.

Competenze

• Current Physical Therapist Assistant (PTA) License to practice in California is required.
• Experience in Home Health care (1 year) is required.
• Current CA Driver’s License.
• Able to travel within a 15 - 25 mile radius on a regular basis.

Mansioni

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Position Overview

We are seeking an experienced, self-driven Full Time Physical Therapist Assistant (PTA) for Health Care to join our dedicated team and provide exceptional care throughout the region. As a PTA, you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.
