Job summary

Interim HealthCare of the Upstate seeks a Home Health Physical Therapist Assistant (PTA) in South Carolina to deliver skilled care in patients' homes, supporting recovery and independence.

You will work under the supervision of a physical therapist, develop and document plans of care, and help prevent hospitalizations while building trusted relationships with patients and families. Competitive pay and benefits are offered.

• Act at the direction and under the supervision of the treating physical therapist and in accordance with the plan of care.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provides services that are ordered by the physician as indicated in the plan of care.
• Assists the treating physical therapist in documenting a patient’s progress and response to treatment.
