Uplift HomeCare, LLC is seeking a Licensed Physical Therapist Assistant (PTA) to provide home health therapy in Lake Wales, FL and surrounding areas. You will work under a licensed PT, delivering prescribed therapy plans in patients' homes with flexible, part-time or PRN scheduling.

This role emphasizes autonomous field work, compassionate patient care, and collaboration with a supportive clinical team to improve mobility and independence for seniors and veterans.

Qualifications

• Active Florida PTA license required.
• Graduate of an accredited PTA program.
• National PTA examination completed.
• Minimum 1 year home health experience must have.
• Current CPR certification required.
• Valid driver’s license and reliable transportation.
• Ability to travel to Lake Wales and surrounding areas.

Responsibilities

• Provide therapy services under the direction of a licensed Physical Therapist.
• Implement prescribed PT interventions and treatment plans.
• Educate patients and caregivers on exercises and home safety.
• Document treatments and patient progress accurately and promptly.
• Monitor patient responses and report changes to supervising PT.

What You’ll Do

As a Home Health Physical Therapist Assistant, you’ll provide therapy services under the direction and supervision of a licensed Physical Therapist, following the established plan of care.

Your responsibilities include:

• Implement prescribed physical therapy interventions and treatment plans.
• Provide therapeutic exercises and activities to improve mobility, strength, balance, coordination, and functional independence.
• Help patients work toward goals established by the supervising Physical Therapist.
• Monitor patient responses to treatment and report changes in condition or concerns to the supervising PT and appropriate clinical personnel.
• Document treatments, patient progress, and responses accurately and promptly.
• Educate patients and caregivers on prescribed exercises, mobility techniques, fall prevention, and home safety.
• Communicate with the supervising PT and other members of the healthcare team to support continuity of care.
• Follow agency policies, professional standards, and applicable documentation requirements.

Qualifications

• Active Florida Physical Therapist Assistant (PTA) license.
• Graduate of an accredited Physical Therapist Assistant program.
• Successful completion of the required national PTA examination.
• Minimum of one year of home health experience required.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong communication, organization, and clinical documentation skills.
• Ability to work independently and travel to patient homes throughout Lake Wales and surrounding areas.
• Commitment to compassionate, safe, and patient-centered care.
