Job summary

Uplift HomeCare, LLC in Lake Wales, FL is seeking a licensed Physical Therapist Assistant (PTA) to provide home health services in Lake Wales and surrounding areas. This field-based role offers flexibility, meaningful patient care, and supportive clinical leadership.

Under the supervision of a licensed Physical Therapist, you’ll implement prescribed interventions, guide patients through exercises, document progress, and ensure safety in the home.

Qualifications

• Active Florida PTA license.
• Graduate of an accredited PTA program.
• Successful completion of the required national PTA examination.
• Minimum of one year of home health experience.

Responsibilities

• Implement prescribed physical therapy interventions and plans.
• Provide therapeutic exercises to improve mobility, strength, balance, and independence.
• Help patients work toward goals set by the supervising PT.
• Monitor patient responses and report changes to the supervising PT.
• Document treatments and patient progress promptly.

• Communicate with the supervising PT and healthcare team to ensure continuity of care.
• Follow agency policies and documentation requirements.

What You’ll Do

As a Home Health Physical Therapist Assistant, you’ll provide therapy services under the direction and supervision of a licensed Physical Therapist, following the established plan of care.

Your responsibilities include:

• Implement prescribed physical therapy interventions and treatment plans.
• Provide therapeutic exercises and activities to improve mobility, strength, balance, coordination, and functional independence.
• Help patients work toward goals established by the supervising Physical Therapist.
• Monitor patient responses to treatment and report changes in condition or concerns to the supervising PT and appropriate clinical personnel.
• Document treatments, patient progress, and responses accurately and promptly.
• Educate patients and caregivers on prescribed exercises, mobility techniques, fall prevention, and home safety.
• Communicate with the supervising PT and other members of the healthcare team to support continuity of care.
• Follow agency policies, professional standards, and applicable documentation requirements.

Qualifications

• Active Florida Physical Therapist Assistant (PTA) license.
• Graduate of an accredited Physical Therapist Assistant program.
• Successful completion of the required national PTA examination.
• Minimum of one year of home health experience required.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong communication, organization, and clinical documentation skills.
• Ability to work independently and travel to patient homes throughout Lake Wales and surrounding areas.
• Commitment to compassionate, safe, and patient-centered care.
