Qualifications

• Active Florida PT license is required.
• Minimum two years PT experience preferred; one year OASIS experience required.
• AHCA Level II background screening is required.
• Current CPR certification is required.
• Valid driver's license and reliable transportation.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work independently while collaborating with an interdisciplinary care team.
• Experience with geriatric or medically complex patients preferred.

Responsibilities

• Evaluate patients and develop treatment plans.
• Provide skilled therapy to improve mobility, function and independence.
• Perform assessments and reevaluations; monitor progress.
• Identify safety concerns and mobility aids.
• Educate patients and caregivers on exercises and safety.
• Communicate progress to physicians and clinical team.
• Coordinate care with nurses, OT, SLP, case managers, and others.
• Complete accurate OASIS documentation when applicable.
• Participate in care conferences and other clinical activities.

What You’ll Do

As a Home Health Physical Therapist, you’ll evaluate patients, develop individualized treatment plans, and provide skilled therapy to improve mobility, function, and quality of life.

Responsibilities include:

• Evaluate patients’ physical abilities, functional limitations, mobility, and rehabilitation needs.
• Develop and revise individualized treatment plans and measurable therapy goals.
• Provide skilled therapeutic exercises, interventions, and appropriate modalities to improve strength, balance, mobility, and functional independence.
• Perform required assessments and reevaluations and monitor patient progress.
• Identify safety concerns, fall risks, and appropriate mobility aids or durable medical equipment.
• Educate patients and caregivers on exercises, safe mobility, fall prevention, and home safety.
• Communicate patient progress, changes in condition, and treatment recommendations to physicians and the clinical team.
• Coordinate care with nurses, occupational therapists, speech therapists, case managers, and other healthcare professionals.
• Complete accurate, timely, and compliant clinical documentation, including required OASIS documentation when applicable.
• Participate in care conferences and other clinical activities as needed.

Qualifications

• Active Florida Physical Therapist license.
• Minimum of two years of PT experience preferred.
• At least one year of OASIS experience required.
• AHCA Level II background screening.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work independently while collaborating with an interdisciplinary care team.
• Compassionate, dependable, and patient-focused approach.
• Experience with geriatric or medically complex patients preferred.
