Job summary

Uplift HomeCare, LLC is seeking an experienced Physical Therapist Assistant to join our home health team serving Riverview, Brandon, and surrounding communities in FL. This part-time role offers flexible scheduling with a pay rate of $60 per visit, with a supportive clinical leadership and opportunities for meaningful patient care in patients' homes.

You will implement treatment plans under a licensed PT, help patients improve mobility and independence, and participate in in-service education

Qualifications

• Active Florida PTA license required.
• Graduation from an accredited PTA program.
• CPR certification required.
• At least 1 year of home health experience.
• Strong communication and documentation skills.

Responsibilities

• Implement physical therapy treatment plans under the direction of a licensed Physical Therapist.
• Provide therapeutic exercises and interventions per established plans.

• Monitor patient responses and report progress to supervising PT.

What You'll Do

As a PTA with Uplift HomeCare, you'll implement physical therapy treatment plans under the direction and supervision of a licensed Physical Therapist, helping patients work toward improved mobility and independence.

Your responsibilities will include:

• Providing therapeutic exercises and interventions according to established treatment plans.
• Helping patients improve strength, balance, mobility, and functional abilities.
• Monitoring patient responses and reporting progress or changes in condition to the supervising PT.
• Educating patients and caregivers on prescribed exercises, safe mobility, and fall prevention.
• Completing accurate, timely, and compliant clinical documentation.
• Communicating effectively with the supervising Physical Therapist and interdisciplinary care team.
• Supporting coordinated care and positive patient outcomes.
• Participating in required in-service education and quality improvement activities.

What We're Looking For

• Active Florida Physical Therapist Assistant (PTA) license.
• Graduation from an accredited Physical Therapist Assistant program.
• Successful completion of applicable licensing examination requirements.
• At least one year of home health experience required.
• Current CPR certification.
• Strong communication, organizational, and documentation skills.
• Valid driver's license, current auto insurance, and reliable transportation.
• Ability to work independently while following the supervising PT's direction.
• Willingness to travel to patient homes throughout Riverview, Brandon, and surrounding areas.
