Job summary

Southwoods Health at Home is seeking a patient-centered Physical Therapist Assistant (PTA) to join our home health team in Boardman, OH. You will deliver PTA services under the supervision of a licensed Physical Therapist in patients' homes to restore function and increase independence.

You will implement prescribed therapies, monitor progress, collaborate with the PT and team, and educate patients and families on home programs. EMR documentation will follow Medicare and facility standards.

Qualifications

• Graduate of an accredited PTA program.
• Active Ohio PTA license.
• Current BLS certification through the American Heart Association.
• Strong interpersonal skills and the ability to provide compassionate care independently in a home setting.

Responsibilities

• Implement medically prescribed PTA treatments according to the plan of care.

About the Role:

Southwoods Health at Home is seeking a patient-centered Physical Therapist Assistant (PTA) to join our growing home health team. In this role, you will provide vital therapy treatments under the direction of a licensed Physical Therapist, helping patients restore function and regain independence in the comfort of their own homes.

• Clinical Care: Implement medically prescribed physical therapy treatments according to the individualized plan of care developed by the Physical Therapist.
• Patient Progress:Monitor and record patient responses to treatment, including strength, motor development, and functional capacity.
• Collaboration: Work closely with the Physical Therapist and the multidisciplinary team to suggest treatment revisions and integrate patient care.
• Education: Empower patients and their families by providing clear instruction on home exercise programs and post-operative recovery protocols.
• Documentation: Maintain accurate, timely patient records in the Electronic Medical Record (EMR) system in accordance with Medicare and facility standards.

What You’ll Need (Qualifications):

• Education: Graduate of an accredited Physical Therapist Assistant program.
• Licensure: Active Physical Therapist Assistant (PTA) license in the State of Ohio.
• Certification: Current BLS certification through the American Heart Association (AHA).
