Job summary

Southwoods Health in Boardman, OH is seeking a Full-Time Outpatient Physical Therapist (PT) or Physical Therapist Assistant (PTA). The role focuses on patient-centered evaluation and treatment, coordinating with the care team to restore function and relieve pain.

The PT/PTA will participate in education for patients and families, supervise trainees as needed, and maintain comprehensive documentation in the electronic health record. Hours are typically Mon-Fri.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio State Physical Therapist licensure or pending license.
• AHA Basic Life Support (BLS) certification required within 30 days of hire.
• Prior outpatient therapy experience preferred.

Responsibilities

• Perform patient-centered evaluations and treatments meeting professional standards.
• Review referrals and medical records; evaluate strength, motor development and functional capacity.
• Develop and revise individualized plans of care to restore function and relieve pain.
• Collaborate with physicians and interdisciplinary team.
• Oversee PTAs, students and unlicensed personnel as applicable.
• Maintain accurate electronic patient records and documentation.

PHYSICAL THERAPIST (PT)

Summary:
Performs patient-centered evaluations and treatments that meet all professional, legal, and regulatory standards.

• Evaluation: Review physician referrals and medical records; perform comprehensive evaluations of strength, motor development, and functional capacity.
• Plan of Care: Develop and revise individualized treatment plans to restore function, relieve pain, and prevent disability.
• Collaboration: Work closely with physicians and the interdisciplinary team to seamlessly integrate therapy into the patient’s overall healthcare plan.
• Supervision: Oversee physical therapist assistants, students, and unlicensed personnel in accordance with state and federal regulations.
• Documentation: Maintain accurate, thorough, and timely electronic patient records.
• Education: Guide patients and caregivers on post-operative recovery, specific physician protocols, and home exercise programs.

Qualifications:

PHYSICAL THERAPIST ASSISTANT (PTA)

Summary:
Performs patient-centered treatments under the direction of a licensed Physical Therapist and according to the established plan of care.

• Treatment: Administer physical therapy treatments to restore function and relieve pain following the supervising PT’s plan of care.
• Communication: Regularly confer with the supervising PT and interdisciplinary care team regarding patient progress and outcomes.
• Documentation: Complete accurate clinical documentation within designated facility timeframes.
• Education: Instruct patients and families on specialized treatment plans, safety techniques, and home exercise programs.
• Professionalism: Maintain scheduling flexibility and actively participate in facility training, staff meetings, and continuing education.

Qualifications:

• Graduate of an accredited Physical Therapist Assistant program.
• Current American Heart Association (AHA) Basic Life Support (BLS) certification.
• Prior experience in an outpatient therapy setting is preferred.
• Status: Full-Time.
