About the Role
Southwoods Health at Home is seeking a patient-centered
Physical Therapist Assistant (PTA) to join our growing home health team. In this role, you will provide vital therapy treatments under the direction of a licensed Physical Therapist, helping patients restore function and regain independence in the comfort of their own homes.
Essential Duties
- Clinical Care: Implement medically prescribed physical therapy treatments according to the individualized plan of care developed by the Physical Therapist.
- Patient Progress: Monitor and record patient responses to treatment, including strength, motor development, and functional capacity.
- Collaboration: Work closely with the Physical Therapist and the multidisciplinary team to suggest treatment revisions and integrate patient care.
- Education: Empower patients and their families by providing clear instruction on home exercise programs and post-operative recovery protocols.
- Documentation: Maintain accurate, timely patient records in the Electronic Medical Record (EMR) system in accordance with Medicare and facility standards.
What You'll Need (Qualifications)
- Education: Graduate of an accredited Physical Therapist Assistant program.
- Licensure: Active Physical Therapist Assistant (PTA) license in the State of Ohio.
- Certification: Current BLS certification through the American Heart Association (AHA).
- Skills: Strong interpersonal skills, clinical problem-solving, and the ability to provide compassionate care independently in a home setting.
