Job summary

Southwoods Health at Home in Boardman, OH, is seeking a patient-centered Physical Therapist Assistant (PTA) to join our home health team on a per diem basis. You will provide vital therapy to patients in their homes under the direction of a Physical Therapist.

You will implement prescribed treatments, monitor progress, collaborate with the care team, educate patients and families, and maintain accurate documentation in compliance with Medicare standards.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Active PTA license in the State of Ohio.
• Current BLS certification through the American Heart Association (AHA).
• Strong interpersonal skills and the ability to provide compassionate care independently in a home setting.

Responsibilities

• Clinical Care: Implement medically prescribed physical therapy treatments according to the individualized plan of care developed by the Physical Therapist.
• Patient Progress: Monitor and record patient responses to treatment, including strength, motor development, and functional capacity.
• Collaboration: Work closely with the Physical Therapist and the multidisciplinary team to suggest treatment revisions and integrate care.
• Education: Empower patients and their families by providing instruction on home exercise programs and post-operative recovery protocols.
• Documentation: Maintain accurate, timely patient records in accordance with Medicare and facility standards.
