Job summary

Lorian Health in Riverside, CA is seeking a Home Health PTA to provide skilled physical therapy interventions for patients at home under the supervision of a licensed Physical Therapist.

You will educate families on home exercise programs, monitor patient progress, ensure safety, and document clinical activities while collaborating with an interdisciplinary care team.

Qualifications

• Graduate of a PTA program approved by the AMA or equivalent.
• Licensed/registered to practice as a PTA in the state of practice.
• Valid driver’s license and insured personal vehicle.

Responsibilities

• Provide skilled physical therapy interventions as ordered by the physician and under supervision of a licensed Physical Therapist.
• Support patients in improving mobility, balance, gait, strength, and functional independence.
• Educate families on home exercise programs and safety strategies.
• Monitor patient condition and communicate changes with the supervising PT and care team.
• Assess the home environment and recommend modifications to promote safety and independence.
• Maintain accurate, timely clinical documentation.
• Collaborate with the interdisciplinary care team and participate in education and in-service programs.

Location: Riverside County

Schedule: Monday thru Friday

As a Home Health PTA, you will:

• Provide skilled physical therapy interventions as ordered by the physician and under the direction and supervision of a licensed Physical Therapist.
• Support patients in improving mobility, balance, gait, strength, and functional independence while helping minimize physical limitations.
• Educate and instruct patients and families/caregivers on home exercise programs, balance and gait training, and strategies to support continued progress.
• Monitor patient condition and progress, communicating changes and concerns with the supervising Physical Therapist, physician, case manager, and care team.
• Assess the home environment and recommend appropriate modifications or strategies to promote patient safety and independence.
• Maintain accurate, complete, and timely clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
• Collaborate with the interdisciplinary care team, participate in education and in-service programs, and provide guidance to other healthcare personnel when appropriate.

Required Qualifications

• Graduate of a physical therapy assistant school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed/registered to practice as a PTA in the state of practice.
• Valid driver’s license and insured personal vehicle (per Lorian requirements).
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possesses and maintains current CPR certification.

This position is based on a pay per visit compensation model. Pay per visit ranges from $50 to $60. Visit is commensurate with experience and depending on type of visit.
