Job summary

Continuum Therapy Partners is hiring a Licensed Physical Therapist Assistant (PTA) for a full-time opportunity at Norwood Crossing, a skilled nursing facility in Chicago, Illinois.

You will provide skilled physical therapy treatments under a licensed PT, assist with gait training, therapeutic exercises, and mobility, while documenting progress and collaborating with the rehab team to support adult and geriatric patients.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Licensed PTA in Illinois is required.

Responsibilities

• Provide skilled physical therapy treatments under supervision of a licensed Physical Therapist.
• Assist patients with gait training, transfer training, therapeutic exercise, balance activities, strengthening, and functional mobility.
• Monitor patient progress and communicate changes to the supervising PT.
• Complete accurate and timely electronic documentation.
• Collaborate with physicians, nursing staff, caregivers, and the rehabilitation team.
• Educate patients and families on mobility, safety, and home exercise programs.
• Maintain compliance with Medicare, state, federal, and company guidelines.

Physical Therapist Assistant (PTA) Responsibilities

• Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist.
• Assist patients with gait training, transfer training, therapeutic exercise, balance activities, strengthening, and functional mobility.
• Monitor patient progress and communicate changes to the supervising Physical Therapist.

• Collaborate with physicians, nursing staff, caregivers, and the interdisciplinary rehabilitation team.
• Educate patients and families on mobility, safety, and home exercise programs.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Illinois Physical Therapist Assistant (PTA) license required.
• Skilled nursing, long-term care, rehabilitation, post-acute care, or outpatient experience is helpful but not required.
• New graduate PTAs are encouraged to apply.
• Strong communication, teamwork, and organizational skills.
