Interim HealthCare - Great Lakes Health Partners is seeking a Home Health Physical Therapist Assistant (PTA) in Green Bay, WI. You will provide home-based physical therapy to patients, work with a supportive home health team, and tailor care plans to individual goals and environments.

The role emphasizes hands-on therapy, education for patients and families, and adherence to home health regulations. CPR certification and WI PTA license required.

Qualifications

• Graduate of an accredited PTA program and active WI PTA license.
• Minimum 1 year PTA experience, ideally home health.
• CPR certification.
• Knowledge of state/federal home health regulations.
• Strong interpersonal skills; compassion and dexterity.

Responsibilities

• Provide home-based physical therapy to improve mobility and function.
• Collaborate with RN/LPN/CNA/HHA/OT/SLP as part of patient’s care team.
• Observe movements, assess progress, document outcomes, and modify care plan.
• Assist with exercises and hands-on therapy to reduce pain and improve mobility.
• Educate patients on care plans, goals, exercises and self-care.
• Coach families on care plan and safe exercise delivery.

As a Home Health Physical Therapist Assistant, heres a big-picture view of what youll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patients plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapist Assistants:

• Graduate of an accredited Physical Therapist Program and active PTA license in WI
• Minimum of 1 years of physical therapy assistant experience, ideally in home healthcare
• CPR certification
