Job summary

Interim HealthCare of Green Bay is seeking a Home Health Physical Therapist Assistant to deliver patient-centered PT in home settings across Green Bay and surrounding areas. You will assess mobility, develop care plans, and collaborate with a supportive home health team to help patients regain strength and independence.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PTA license in Wisconsin.
• Minimum of 1 year PTA experience, ideally in home health care.
• CPR certification is required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgement and interpersonal skills; compassion and dexterity are valued.

Responsibilities

• Provide home-based physical therapy to patients with mobility challenges.
• Collaborate with a home health team including RN, LPN, CNA, HHA, OT and SLP.
• Observe movements, assess progress, and adjust the care plan as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on care plans, goals, and safety during exercises.
• Coach families on therapy goals and safe home exercise execution.
