Job summary

Interim HealthCare is seeking a Home Health Physical Therapist (PT) in Janesville, WI and surrounding areas. You will provide one-on-one, home-based physical therapy to help patients regain strength and mobility in their daily lives.

You will work with an integrated home health team, document progress, adjust plans, and educate patients and families. This role requires a WI PT license, CPR, and 1 year of PT experience, with mileage reimbursement and growth opportunities.

Qualifications

• Graduate of an accredited Physical Therapist program and active Wisconsin PT license.
• Minimum 1 year of physical therapy experience, preferably home health.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide home-based physical therapy to regain mobility and function.
• Collaborate with a home health team to meet patient goals.
• Observe movements, document progress, and adjust care plans.
• Assist with exercises and hands-on therapy to reduce pain.
• Educate patients and families on the care plan and exercises.
• Coach families on safety and therapy goals at home.

As a Home Health Physical Therapist, heres a big-picture view of what youll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patients plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in Wisconsin
• Minimum of 1 year of physical therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
