Job summary

Ultimate Therapy is seeking a motivated Physical Therapist Assistant (PTA) to join our rehabilitation team at Autumn Lake in Windsor, CT. This full-time role offers the chance to work closely with experienced clinicians to help residents improve mobility, strength, and independence in a skilled nursing setting.

Applicants must hold an active Connecticut PTA license and have strong communication and teamwork skills.

Qualifications

• Active Connecticut PTA license required.
• Experience in skilled nursing or rehabilitation preferred but not required.
• New graduates are encouraged to apply.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist.
• Help residents improve strength, balance, gait, endurance, and functional mobility.
• Carry out individualized treatment plans based on resident needs and goals.
• Monitor progress and communicate updates to the supervising Physical Therapist.
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies.
• Maintain accurate and timely clinical documentation.
• Collaborate with PT, OT, SLP, nursing, and other members of the care team.

Ultimate Therapy is seeking a motivated Physical Therapist Assistant (PTA) to join our rehabilitation team. This is a great opportunity to work alongside experienced clinicians while helping residents improve mobility, rebuild strength, and achieve greater independence in a skilled nursing and post-acute care setting.

What You’ll Do
