Job summary

Interim HealthCare is seeking a Home Health Physical Therapist Assistant in Michigan. The PTA collaborates with the clinical team to assess patients, implement the plan of care, document progress, and assist with discharge planning.

This role offers part-time, per-visit pay with mileage reimbursement, a tablet with Wi-Fi, paid training, flexible assignments, weekly pay, and ESOP eligibility after 1,000 hours. Active Michigan PTA license and CPR required; home health experience is a plus.

Qualifications

• Current licensure as a PTA in Michigan.
• 2+ years of PTA experience in the last 5 years.
• Active CPR certification is required.
• Home health experience is a plus.

Responsibilities

• Participates in ongoing interdisciplinary assessment of the patient.
• Acts under the direction of the treating PT and plan of care.
• Participates in development and evaluation of the plan of care with patient and caregivers.
• Provides services ordered by the physician as indicated in the plan of care.
• Documents patient progress and response to treatment accurately and timely.
• Educates patient and family to promote progress toward goals.
• Communicates with health care team to coordinate care and discharge planning.
