Job summary

Interim HealthCare of NE Florida is seeking a Home Health Physical Therapist Assistant in Deltona, FL to deliver compassionate, patient-centered care in patients' homes. The role emphasizes hands-on therapy, patient education, and collaboration with a local care team to achieve meaningful mobility goals.

Ideal candidates hold a Florida PTA license, CPR certification, and have at least 1 year of PTA experience, preferably in home health.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program and active Florida PTA license.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Minimum 1 year PTA experience, preferably in home health.
• Strong clinical judgement and compassionate patient care.

Responsibilities

• Provide home-based physical therapy to patients in Deltona and surrounding areas.
• Observe movements, conduct assessments, document progress and modify care plans.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on plan of care, goals, exercises, and safe self-care.
• Collaborate with home health team to meet patient goals and ensure quality care.
