Job summary

Interim HealthCare is seeking a Home Health Physical Therapist Assistant (PTA) to support patients in their home health journey. The PTA will participate in assessments, work under the direction of the treating PT, and contribute to the plan of care while maintaining thorough documentation.

This role emphasizes patient-centered care and collaboration with the healthcare team. Qualified candidates hold an active Michigan PTA license, CPR certification, and prior Home Health experience is a plus.

Qualifications

• Active Michigan PTA license.
• Two years of PTA experience in the last five years.
• Current CPR certification.

Responsibilities

• Participates in interdisciplinary assessment.
• Assists PT in plan of care development and evaluation.
• Documents patient progress and interventions clearly and timely.
• Educates patient and family to promote progress and goals.
• Communicates with the care team to coordinate discharge planning.
