Job summary

Prncares is seeking a compassionate Physical Therapist Assistant (PTA) for home health in the Madison area. The role supports independent practice with a clinical team behind you, delivering PT services in patients’ homes under the plan of care.

The PTA must be a graduate of an accredited program, hold Wisconsin PTA licensure, and maintain CPR certification. Flexible, full-time position with benefits and mileage reimbursement in the Madison area.

Qualifications

• CPR certification required
• Graduate of an accredited Physical Therapist Assistant program

Responsibilities

• Deliver Physical Therapy treatments according to the plan of care developed by the Physical Therapist
• Collaborate with the interdisciplinary team to identify and obtain patient-specific DME and adaptive equipment
• Maintain accurate daily treatment records and progress notes
• Educate patients and caregivers on safe discharge planning

KeyResponsibilities

• Deliver Physical Therapy treatments according to the plan of care developed by the Physical Therapist
• Collaborate with the interdisciplinary team to identify and assist with obtaining patient‑specific DME (mobility aids, wheelchairs, cushions, orthotics, adaptive equipment, etc.)
• Maintain accurate daily treatment records and progress notes as directed by the Physical Therapist
• Work with the Physical Therapist and clinical team to educate patients and caregivers on safe discharge planning

Requirements

• Graduate of an accredited Physical Therapist Assistant program
• Current Wisconsin PTA license/certification
• Current American Heart Association CPR certification
