Job summary

PRN Home Health & Therapy invites a compassionate Physical Therapist Assistant (PTA) to join our home health team in Marinette, WI. This 0.2 FTE role supports independent patient care within a collaborative clinical setting and offers growth opportunities and a competitive benefits package.

Responsibilities include delivering therapy per the plan of care, assisting with DME procurement, documenting progress, and educating patients and caregivers on safe discharge planning.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Wisconsin PTA license/certification.
• Current American Heart Association CPR certification.

Responsibilities

• Deliver Physical Therapy treatments according to the plan of care developed by the Physical Therapist.
• Collaborate with the interdisciplinary team to identify and assist with obtaining patient‑specific DME (mobility aids, wheelchairs, cushions, etc.).
• Maintain accurate daily treatment records and progress notes as directed by the Physical Therapist.
• Work with the Physical Therapist and clinical team to educate patients and caregivers on safe discharge planning.

Key Responsibilities
