Continuum Therapy Partners is seeking a Home Health Physical Therapist Assistant to join our Stevens Point, WI team. We offer full-time and PRN positions with the flexibility to create your own schedule and provide one-on-one patient care.

Qualified candidates hold an Associate Degree in PTA, an active Wisconsin PTA license, and have strong clinical and communication skills. We support growth with licensure reimbursement, benefits, and a collaborative team environment.

Qualifications

• Associate degree required.
• Current, unencumbered Wisconsin PTA license.
• 1 year of PTA experience preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities:

• Associate Degree in Physical Therapist Assistant Program required
