PHYSICAL THERAPIST ASSISTANT (PTA) SHEDULE:

Full-Time, 5 days per week, Monday-Friday, 8:30 AM-5:00 PM

TERRITORY: Darke County

AGENCY: Kettering Home Care

As a Home Health Physical Therapist (PT), you will provide skilled physical therapy services under physician orders to help patients regain mobility, restore function, reduce pain, and maximize independence in the comfort of their homes.

HOW YOU'LL WORK:

You’ll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensure the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

MAJOR AREAS OF RESPONSIBILITY:
  • Patient Care: Provides skilled services in a patient’s home as developed by the Physical Therapist plan of care. Observes, records, and reports patient reactions, changes, and progress. Performs physical therapy procedures and diagnostic test on patients for whom they have been medically prescribed. Treats patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc. Assesses functional problems and provided intervention and, in conjunction with the physical therapist, works toward goals to improve patient functional status. Communicate effectively with physicians, therapists, and coworkers. Participates in case conferencing promoting coordination of care. Recommend prosthetic/orthotic devices for patients or evaluate their device and make minor adjustments or recommend modification. Educates staff in carrying out specific physical therapy modalities when appropriate.
  • Patient & Family Education: Educates patient and family on therapy techniques involved, safety measures, and supportive activities. Instructs patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices. Listen to patients and family members. Fosters independence in patients and their families to prevent dependence on the agency.
  • Care Coordination: Collaborate with physicians, nurses, PT, and other interdisciplinary team members to ensure coordinated, high-quality care. Communicate patient progress, changes in condition, and scheduling updates with the care team.
  • Documentation & Compliance: Complete accurate and timely documentation in accordance with agency policies, Medicare guidelines, and state and federal regulations. Documents all aspects of treatment, assessments, and patient education. Completes chart audits at agency location as requested.
  • Professional Responsibilities: Attend mandatory meetings, in-service education, and continuing education programs. Assist with onboarding and mentoring new clinicians as needed. Maintain a professional appearance and represent the organization positively. Follow all agency policies, procedures, and standards of practice. Participate in case conferences, interdisciplinary meetings, and on‑call responsibilities as assigned.

  • Home care experience preferred.
REQUIREMENTS:
  • Physical Therapist Assistant (PT) with current license in the state of employment.
  • Minimum one-year experience as a Physical Therapist Assistant (PTA) in an acute care setting.
  • Valid driver’s license and auto insurance in your name as a driver.
  • Capable of all physical demands.
