Alternate Solutions Health Network in Cincinnati, OH seeks a Physical Therapist I to provide comprehensive skilled services in patients' homes under physician orders. You will educate patients and families, perform OASIS assessments, develop care plans, and coordinate with care teams while maintaining compliance with Medicare guidelines.
The role requires licensure as a Physical Therapist in the state of employment and a valid driver's license for travel to patient locations.

The Physical Therapist I (PT) provides comprehensive skilled services as ordered by the attending physician. The PT educates patients and their family members and ensures the safety of the patient. The PT I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.
