Job summary

Interim HealthCare of the Twin Cities in Roseville, MN is seeking a Licensed Home Care Physical Therapist Assistant (PTA) to provide in-home care for patients in the southwest metro, with hours of 8:00am to 5:00pm, Monday through Friday. Under the supervision of a licensed physical therapist, you will assess and treat impairments, document progress, and educate patients and families.

You must hold an active MN PTA license, have at least two years of PTA experience, a valid driver s license, and

Qualifications

• Active Minnesota PTA license in good standing.
• Two years PTA experience.
• Valid drivers license, reliable transportation, and proof of auto insurance.
• CPR/BLS certification.

Responsibilities

• Provide services to treat impairments and functional changes under the supervision of a licensed physical therapist.
• Participate in interdisciplinary patient assessments.
• Provide services ordered by the physician as indicated in the plan of care.
• Complete timely, legible documentation of treatment, patient response, and progress toward goals.
• Counsel and educate the patient, family, and caregivers to promote progress toward goals.

Hours: Full-time | Monday - Friday | 8:00am - 5:00pm

Territory : This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position would be the southwest metro.

Pay and Benefits:

• Hourly Pay Range: $31.40 - $37.44

A few must-haves for this position:

• Currently licensedas a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
• Two (2) years of experience as a PTA.
• Able to travel within geographic area serviced by the office.Valid driver'slicense, reliable transportation, and proof of auto insurance.
• CPR/BLS Certification

Here are some functions of what you would do as a Licensed Physical Therapist Assistant:

• Provide services for treating impairments, functional limitations, disabilities or changes in physical function under the direction and supervision of a qualified physical therapist.
• Participates in ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Completes and submits timely, legible, accurate, relevant and sufficient documentation addressing the treatment and interventions provided, the patient's response to the treatment and interventions, and the patient's progress toward goals in accordance with professional standards of practice, policy and procedures, and payer requirements.
• Counsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
