Job summary

PACS Group is seeking a compassionate Physical Therapist Assistant (PTA) to join Kern River Transitional Care on a PRN basis in California. You will work under the supervision of a licensed Physical Therapist to help residents improve mobility and independence.

Responsibilities include providing treatments, guiding exercises, documenting progress, and educating residents and families on safety techniques. PRN scheduling offers flexibility in a supportive team environment.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current California PTA license in good standing.
• Current CPR certification.
• Previous skilled nursing or long-term care experience preferred.
• Strong communication, documentation, and interpersonal skills.

Responsibilities

• Provide physical therapy treatments according to the plan of care established by the Physical Therapist.
• Assist residents with therapeutic exercises, mobility training, transfers, balance, and gait activities.
• Observe and document residents’ progress, responses, and changes in condition.
• Communicate resident updates and concerns to the supervising Physical Therapist.
• Educate residents, families, and caregivers on exercises and safety techniques.
• Maintain accurate and timely clinical documentation.
• Follow all facility policies and applicable state and federal regulations.
• Collaborate with nursing, rehabilitation, and interdisciplinary team members.
• Promote a safe, supportive, and resident-centered environment
