Job summary

Kern River Transitional Care in California is seeking a compassionate Part-Time Physical Therapist to join our rehabilitation team. You will evaluate residents, design individualized plans, provide skilled PT services to improve mobility, safety, and independence, and help residents regain the ability to walk and transfer.

Work alongside physicians, nurses, and therapists in an interdisciplinary setting. Excellent communication, documentation, and time-management are essential.

Qualifications

  • Graduate of an accredited physical therapy program.
  • Current California Physical Therapist license is required.
  • Current CPR/BLS certification required.

This is an excellent opportunity to work in a supportive environment where your clinical expertise will directly improve residents’ mobility, confidence, independence, and quality of life.Our interdisciplinary team is committed to delivering personalized, resident-centered care and celebrating every milestone along the road to recovery.### Essential Duties and Responsibilities* Evaluate residents’ mobility, strength, balance, coordination, range of motion, and functional abilities.* Develop individualized treatment plans based on each resident’s needs, goals, and medical condition.* Provide skilled physical therapy services to improve mobility, independence, safety, and overall function.* Help residents regain the ability to walk, transfer, maintain balance, and complete daily activities safely.* Establish measurable goals and modify treatment plans based on residents’ progress and changing needs.* Provide therapeutic exercise, gait training, balance activities, transfer training, and other appropriate interventions.* Recommend and train residents in the safe use of walkers, wheelchairs, canes, and other adaptive equipment.* Educate residents, families, caregivers, and facility staff regarding treatment plans, safety, positioning, and mobility techniques.* Complete evaluations, progress notes, discharge summaries, and other required documentation accurately and on time.* Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.* Participate in resident care conferences, discharge planning, and quality-improvement activities.* Supervise Physical Therapist Assistants and rehabilitation support staff as permitted by law and facility policy.* Maintain residents’ privacy and the confidentiality of all health information.* Follow all professional standards, facility policies, safety procedures, and regulatory requirements.* Perform additional duties as assigned.### Qualifications* Graduate of an accredited physical therapy program.* Current and unrestricted California Physical Therapist license required.* Current CPR/BLS certification required.* Skilled nursing, rehabilitation, long-term-care, or geriatric experience preferred.* New graduates are encouraged to apply.* Strong clinical assessment, communication, documentation, and time-management skills.* Ability to work independently and collaboratively as part of an interdisciplinary team.* Knowledge of PointClickCare or another electronic medical record system preferred.* Compassionate and resident-centered approach to care.### Why Join Kern River Transitional Care?* Competitive pay of $65.00–$70.00 per hour, based on experience* Full-time employment* Medical, dental, and vision insurance* 401(k) retirement plan* Life insurance* Paid time off and paid holidays* Continuing education and professional development opportunities* Supportive rehabilitation and leadership teams* Reward and bonus opportunities* Opportunities for career growth* Meaningful work with the ability to see residents make measurable progress*Benefits are subject to eligibility requirements and facility policy.***If you are a dedicated Physical Therapist who is passionate about helping residents regain their strength and independence, apply today and grow your career with Kern River Transitional Care!**We are an equal opportunity employer.
