Sierra Home Health is seeking a Physical Therapist Assistant (PTA) to join our compassionate home health team. Youll provide in-home therapy under a licensed PT, with travel to Truth or Consequences available 1–2 days per week and mileage reimbursement.

Qualifications

• Current NM PTA licensure required.
• Home health experience preferred but not required.
• Ability to work independently in patients' homes.
• Reliable transportation and valid driver's license.
• Willingness to travel to Truth or Consequences 1–2 days per week.

Responsabilités

• Deliver in-home physical therapy services under supervision of a licensed Physical Therapist.
• Travel to Truth or Consequences 1–2 days per week with mileage compensation.
• Carry out treatment plans to help patients regain mobility, function, and independence.
• Communicate progress and changes in patient condition to the supervising therapist.
• Educate patients and caregivers on exercises and safety practices.
• Document visits and progress notes accurately and on time.
• Participate in care team meetings and interdisciplinary planning.
• Uphold infection control and safety standards during all home visits.
• Maintain professional rapport with patients and families.

Key Responsibilities:

• Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist
• Willingness to travel to Truth or Consequences one to two days a week with mileage compensation
• Carry out treatment plans to help patients regain mobility, function, and independence
• Communicate progress and changes in patient condition to the supervising therapist

Requirements

What We’re Looking For:

• Current licensure as a Physical Therapist Assistant in New Mexico
• Previous experience in home health preferred, but not required
• Strong interpersonal and communication skills
• Confidence working independently in the field
• Adaptability to changing schedules and home environments
• Commitment to quality, safety, and patient-centered care
• Reliable transportation and valid driver's license
• Occasional Travel to T or C preferred
