Job summary

Sierra Home Health and Hospice seeks a dedicated Physical Therapist Assistant to join our home-based care team. You will deliver in-home therapy under a supervising PT, with travel to Truth or Consequences as needed and mileage reimbursed.

You will educate patients, document progress, and collaborate with the care team to support patient independence and safety. We value compassionate care, autonomy in the field, and flexible scheduling within a community-focused nonprofit healthcare setting.

Qualifications

• License to practice as PTA in NM.
• Experience in home health is preferred but not required.
• Excellent communication and teamwork skills.
• Ability to work independently in field settings.

Responsibilities

• Deliver in-home physical therapy services under supervision of a licensed PT.
• Occasionally travel to Truth or Consequences (T or C) 1–2 days per week with mileage reimbursement.
• Implement treatment plans to improve mobility and independence.
• Communicate patient progress to supervising therapist.
• Educate patients and caregivers on exercises and safety.
• Document visits and progress notes on time.
• Participate in care team meetings and plan care with interdisciplinary team.
• Maintain infection control and safety during visits.
• Build a compassionate rapport with patients and families.

About the Role:

Sierra Home Health is seeking a Physical Therapist Assistant (PTA) to join our compassionate and collaborative home health team. Key Responsibilities:

• Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist
• Willingness to travel to Truth or Consequences one to two days a week with mileage compensation
• Carry out treatment plans to help patients regain mobility, function, and independence
• Communicate progress and changes in patient condition to the supervising therapist
• Educate patients and caregivers on exercises and safety practices
• Document visits and progress notes accurately and on time
• Participate in care team meetings and interdisciplinary planning
• Uphold infection control and safety standards during all home visits
• Maintain professional, compassionate rapport with patients and families

Requirements

What We’re Looking For:

• Current licensure as a Physical Therapist Assistant in New Mexico
• Previous experience in home health preferred, but not required
• Strong interpersonal and communication skills
• Confidence working independently in the field
• Adaptability to changing schedules and home environments
• Commitment to quality, safety, and patient-centered care
• Reliable transportation and valid driver’s license
• Occasional Travel to T or C preferred
