Sierra Home Health & Hospice is seeking a Physical Therapist Assistant to join our compassionate home health team in New Mexico. You will deliver in-home therapy, travel to Truth or Consequences 1–2 days weekly with mileage reimbursement, and support patients in regaining mobility and independence.

We value autonomy, strong communication, and a patient-centered approach, with flexible scheduling and a supportive leadership team that helps you grow within a nonprofit healthcare organization.

Competenze

• Current licensure as a Physical Therapist Assistant in New Mexico.
• Home health experience preferred but not required.
• Strong interpersonal and communication skills; able to work independently.

Mansioni

• Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist.
• Travel to Truth or Consequences 1–2 days per week with mileage compensation.
• Carry out treatment plans to help patients regain mobility, function, and independence.
• Communicate progress and changes in patient condition to the supervising therapist.
• Educate patients and caregivers on exercises and safety practices.
• Document visits and progress notes accurately and on time.
• Participate in care team meetings and interdisciplinary planning.
• Uphold infection control and safety standards during all home visits.
• Maintain professional, compassionate rapport with patients and families.
