Job summary

Harmony Home Health & Hospice is seeking a Home Health Physical Therapist Assistant to provide physician-prescribed PT services in patients’ homes. You will deliver evidence-based interventions, coordinate care with a diverse team, and document promptly in the EMR under the supervision of a PT.

Qualifications include an active PTA license and CPR with BLS, plus a valid driver’s license and ability to work independently in home settings.

Qualifications

• Active State Physical Therapist Assistant license.
• Current CPR with BLS card.
• Valid driver’s license with minimum auto insurance.
• Knowledge of federal, state, and local laws governing home health operations.
• Strong organizational skills and ability to work without direct supervision.
• Ability to remain calm in emergencies or crises.
• Computer literacy and EMR documentation experience.

Responsibilities

• Provide physician-prescribed physical therapy under a plan of care established by the PT.
• Provide evidence-based direct intervention with patients in their home.
• Communicate effectively with a diverse patient population and co-workers.
• Work under supervision of a PT: communicate concerns, discuss treatment changes, and participate in patient/family education as delegated.
• Submit required documentation in a timely manner.

Responsibilities

• Provide physician-prescribed physical therapy under a plan of care established by the PT
• Provide evidence-based direct intervention with patients in their place of residence
• Communicate effectively and respectfully with a diverse patient population and co-workers
• Work under the supervision of a PT:
  • Promptly communicating any patient concerns or changes in status
  • Consulting with PT with regard to suggested changes in treatment
  • Participating in patient and family education as delegated by the PT
• Submit required documentation in a timely manner

Qualifications

• Valid driver’s license, at least state minimum auto insurance, and an operational vehicle
• Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing home health operations

• Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system

Physical Requirements

• Duties require extensive standing, walking, and sitting
