Continuum Therapy Partners, LLC is seeking a Physical Therapist Assistant to join Avalon Health & Rehab in Mt. Vernon, Ohio.

The role focuses on implementing individualized treatment plans under the supervision of a licensed Physical Therapist and supporting patient progress in daily care. Candidates must have an Associate Degree in Physical Therapist Assistant and an active PTA license in Ohio; one year of PTA experience is preferred.

Qualifications

• Associate Degree in Physical Therapist Assistant Program is required.
• Current, unencumbered state Physical Therapist Assistant License.
• 1 year of previous experience as a Physical Therapist Assistant preferred.
• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
