Continuum Therapy Partners is seeking a dedicated Physical Therapist Assistant to join our Oak View Health and Rehabilitation Center in Orange Park, FL. The role focuses on implementing personalized treatment plans under a PT, ensuring patient progress is tracked and communicated.

You will educate patients on home exercises, use of assistive devices, and participate in a team-driven, patient-centered care approach.

Qualifications

• Associate degree in Physical Therapist Assistant Program required.
• Current, unencumbered state PTA license.
• 1 year of PTA experience preferred.

Responsibilities

• Implements individualized treatment plans under supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities:

• Associate Degree in Physical Therapist Assistant Program required


• Current, unencumbered state Physical Therapist Assistant License


• 1 year of previous experience as a Physical Therapist Assistant preferred


• Implements individualized treatment plans under the supervision of a Physical Therapist
