Continuum Therapy Partners in Jacksonville, FL is seeking a full-time Physical Therapist Assistant to support a therapist-led care model across our expanding network. The position involves implementing individualized treatment plans under supervision, educating patients on home programs, and documenting progress to help the supervising PT adjust care.

Formación

• Associate Degree in Physical Therapist Assistant Program required.
• Current, unencumbered state Physical Therapist Assistant License.
• 1 year of previous experience as a Physical Therapist Assistant preferred.
• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities:

• Associate Degree in Physical Therapist Assistant Program required
