Job summary

Remedy Home Health Care in Orange County is seeking a dedicated Physical Therapist Assistant (PTA) for home health, per diem. You will deliver one-on-one therapy in patients’ homes, helping them regain mobility and independence with a focus on fall prevention and functional recovery.

The role requires an active California PTA license and CPR certification, with 1 year of clinical experience preferred. Per diem options offer flexible schedules and the chance to impact community-based senior care

Qualifications

• Active California PTA license is required.
• Current CPR certification (AHA) required.
• Minimum 1 year of clinical experience; home health experience preferred.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises, mobility training, and rehab programs.
• Support seniors in improving strength, balance, and independence.
• Monitor, document, and report patient progress to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, safety, and fall prevention.
• Collaborate with physicians, RNs, and the interdisciplinary care team.
• Complete documentation using WellSky Kinnser EMR.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:
