Remedy Home Health Care is seeking a dedicated Physical Therapist Assistant (PTA) to join our West Los Angeles home health team. You will deliver one-on-one therapy in patients' homes, focusing on rehabilitation, mobility and independence.

Qualifications

• Active California PTA license.
• Current CPR certification (AHA).
• Minimum 1 year of clinical experience; home health preferred.
• Strong interest in home health, rehab and senior care.
• Excellent communication and organizational skills.

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises and mobility training.
• Support senior care patients in improving strength, balance and independence.
• Monitor, document and report progress to the supervising PT.
• Educate patients and caregivers on home exercises, safety and fall prevention.
• Collaborate with physicians, RNs and the care team.
• Complete documentation using EMR systems.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs
• Support senior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation using WellSky Kinnser EMR

Qualifications:

• Active California Physical Therapist Assistant (PTA) license
• Current CPR certification (AHA required)
• Minimum 1 year of clinical experience (home health experience preferred)
• Strong interest in home health, rehab, and senior care
• Excellent communication and organizational skills
