Job summary

Remedy Home Health Care is actively hiring a dedicated Physical Therapist Assistant (PTA) to join our home health team in West Los Angeles County. The PTA will deliver one-on-one care in patients' homes to improve mobility, rehabilitation, and independence.

Requirements include California PTA license and CPR certification, with a minimum of 1 year of clinical experience; home health experience preferred. Per diem, part-time, or full-time options with a sign-on bonus are available.

Qualifications

• Active California PTA license required.
• CPR certification (AHA) required.
• Minimum 1 year of clinical experience; home health preferred.
• Strong interest in home health and senior care.

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs.
• Support senior care patients in improving strength, balance and independence.
• Monitor, document, and report patient progress to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, safety and fall prevention.
• Collaborate with physicians, RNs, and the interdisciplinary care team.
• Complete documentation using WellSky Kinnser EMR.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:
