Job summary

Silver Creek Physical Therapy, a member of Therapy Partners Group, is seeking an Outpatient Physical Therapist Assistant to join our dedicated team in San Jose.

You will support licensed therapists with therapeutic exercises, modalities, and patient education, while documenting progress in our Raintree system.

Applicants should have an Associate Degree in Physical Therapy Assistant, current licensure, and a collaborative, patient-centred approach; outpatient experience is preferred.

Qualifications

• Associate Degree in Physical Therapy Assistant from an accredited institution.
• Current state licensure or eligibility for licensure as a Physical Therapist Assistant.
• Previous outpatient experience preferred but not required.

Responsibilities

• Patient Follow-Up: Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Treatment Planning: Assist physical therapists in developing and implementing patient treatment plans.
• Therapeutic Interventions: Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: While using Raintree, monitor and document patient progress during treatment sessions and report changes to the physical therapist.
• Patient Education: Educate patients and families on exercises, pain management techniques, and care plans.
• Professional Atmosphere: Work collaboratively with the healthcare team to ensure optimal patient care and safety.
